Game Description
Pathologic 2 is a first-person survival RPG and psychological horror game about a plague consuming an isolated rural town. The current PC storefront name remains Pathologic 2 on Steam and GOG; there is no verified replacement title or definitive rebrand to confuse with Pathologic Classic HD. Developed by Ice-Pick Lodge and published by tinyBuild, it is a reimagining of the original Pathologic rather than a conventional sequel that assumes you have completed the earlier game.
You play as Artemy Burakh, the Haruspex, a surgeon returning to a town where his father has been murdered and a deadly outbreak is spreading through the population. The story mixes medical investigation, surreal drama, social collapse, and difficult choices. It is deliberately hostile to the idea that every problem has a clean solution.
A survival loop ruled by time
Pathologic 2 takes place across 12 in-game days, with events, conversations, deaths, and opportunities changing as the clock advances. Each day is a race between urgent objectives, travel through infected districts, medical work, trading, scavenging, and the need to rest. There is never enough time to pursue every lead.
The town is an interconnected open environment viewed from a first-person perspective. You walk between districts, search homes and abandoned buildings, speak with important characters, examine clues, and choose which appointments or emergencies deserve priority. A journey that looks short on the map can become expensive when thirst, exhaustion, combat, or a sudden story event interrupts the route.
Survival depends on balancing hunger, thirst, exhaustion, health, immunity, and infection. Food and clean water keep Artemy functional, while sleep restores exhaustion at the cost of valuable hours. Entering plague zones, approaching infected people, consuming unsafe items, or mishandling medicine can push immunity toward a dangerous infection.
Scarcity is not limited to a conventional loot system. The town's economy changes as the outbreak worsens, and useful supplies may be obtained through barter, exploration, medical work, theft, or violence. Small objects can become meaningful trade goods, while food, medicine, weapons, and crafting materials compete for limited inventory space.
Medicine, autopsies, and choices without guarantees
Artemy is more than a survivor with a health bar. He can treat patients, perform autopsies, collect biological materials, brew tinctures, and work toward a cure for the Sand Pest. Medical knowledge gradually becomes a practical tool for deciding who can be helped, which remedies are worth preparing, and how much risk to accept in contaminated areas.
Crafting supports the medical and survival systems rather than turning the game into a conventional equipment treadmill. Herbs, organs, laboratory ingredients, and scavenged materials can be processed into tinctures, medicines, or useful items. The best results often require planning several steps ahead, because brewing, searching, and traveling all consume time that cannot be recovered.
Combat is present but intentionally brief, awkward, and dangerous. Knives, improvised weapons, firearms, and defensive movement can help against muggers and hostile creatures, yet fighting is rarely the most efficient answer. A wounded Artemy loses more than health: combat can cost stamina, supplies, reputation, and the time needed to reach someone before their situation changes.
Reputation is tracked across the town and affects how people respond to Artemy. Looting, stealing, killing, or failing to help can change local relationships, while trade and assistance may open safer or more useful interactions. The result is a social survival system where the town remembers actions even when the player wishes to treat each encounter as isolated.
The narrative also treats failure as part of the structure. Death, missed appointments, and lost characters do not simply reset the world into a perfect state. Pathologic 2 asks you to continue under compromised conditions, learn from what went wrong, and accept that a successful run is not the same thing as saving everyone.
What the PC version includes
The standard PC game is a single-player experience focused on the Haruspex route. It is not a cooperative campaign, does not offer split-screen play, and is not built around competitive multiplayer. The main progression comes from exploring the town, following narrative threads, treating patients, managing survival meters, and making decisions under pressure.
Steam lists Pathologic 2: Marble Nest as separate additional content. Marble Nest is a standalone story scenario set in the same world, while the other listed add-ons are the Artbook and Soundtrack. The base game is a paid title rather than a free-to-play product, and there is no season-pass structure required to understand the main campaign.
The title can be confusing for players comparing editions. Pathologic Classic HD is the enhanced PC edition of the original game, while Pathologic 2 is the newer reimagining centered on Artemy. Current storefront listings continue to use Pathologic 2, so searching for that exact name is the clearest way to find the intended PC version.
Who should play Pathologic 2?
Pathologic 2 is a strong fit for players who want oppressive resource management, unreliable information, branching consequences, and a story that treats discomfort as a design tool. It rewards careful observation, route planning, experimentation, and the willingness to let some objectives fail.
It is a poor match for players who expect smooth action combat, abundant supplies, clear quest markers, conventional heroic progression, or a forgiving survival sandbox. The game can feel unfair when first approached as a normal RPG, because its central challenge is not becoming powerful enough to solve every problem. The challenge is deciding what cannot be solved today.
Players who enjoy systemic first-person exploration may find useful points of comparison in immersive-sim survival games such as Prey, although Pathologic 2 is less focused on combat builds and more interested in social pressure, illness, and irreversible trade-offs.
PC features and storefront differences
On Steam, the game includes Steam Achievements, Steam Trading Cards, Family Sharing, and Remote Play on TV. Steam lists English and Russian for the interface, full audio, and subtitles. No broader localization should be assumed when choosing the PC edition.
The GOG version is listed as DRM-free and does not require GOG Galaxy to play. Steam and GOG therefore offer different launcher expectations, but both storefronts use the same Pathologic 2 name rather than presenting separate modern editions.
The game is designed around solo keyboard-and-mouse-style first-person exploration, but the storefront information does not present a prominent official controller-support designation. Players specifically seeking verified gamepad support should check the selected storefront's current input information before purchase rather than treating Remote Play on TV as a controller guarantee.
For another bleak first-person experience built around environmental investigation, resource pressure, and hostile spaces, story-driven survival horror like SOMA offers a more linear structure and a different approach to vulnerability. Players looking for broader exploration and faction tension can also compare it with open-area survival shooters such as the S.T.A.L.K.E.R. Trilogy, though Pathologic 2 replaces firearms and artifact hunting with medicine, barter, and social consequence.
Pathologic 2 FAQ
Is Pathologic 2 a sequel or a remake?
Pathologic 2 is best understood as a reimagining of the original Pathologic. It retells and expands the Haruspex story with redesigned systems, presentation, and survival mechanics. You do not need to complete the original before starting it.
Is Pathologic 2 still the current PC storefront name?
Yes. Steam and GOG currently list the game as Pathologic 2. Pathologic Classic HD refers to the earlier game, not a renamed edition of Pathologic 2.
What is the main gameplay structure?
The campaign is organized around 12 in-game days. You investigate the plague, follow character-specific objectives, gather supplies, perform medical work, explore districts, and decide how to spend limited time. It is a mastery loop built around learning the town and making better priorities on later attempts, not a repetitive score-attack mode.
Does Pathologic 2 have co-op or multiplayer?
No. It is a single-player game. Steam lists Remote Play on TV, but that feature does not turn the campaign into local or online co-op.
What languages are supported on PC?
Steam lists English and Russian for the interface, full audio, and subtitles. The GOG listing also identifies English and one additional language, but players should confirm the exact storefront language table before purchase if localization is important.
What does Marble Nest add?
Marble Nest is separate additional content that presents a standalone Pathologic story scenario. It is not required for the main Haruspex campaign, and the other listed add-ons are the Artbook and Soundtrack rather than additional playable routes.
