Mafia: The Old Country (also referred to as Mafia 4) is the fourth entry in the Mafia series and a prequel set in early-1900s Sicily. You’re not here to tick off map icons for hours. This time, the game goes for a linear structure and moves you through different corners of the island as the story unfolds.
What you do in Mafia: The Old Country
You step into the shoes of Enzo. His life starts rough - he grew up working in a sulfur mine. Looking for something better, he ends up in the orbit of Don Torrisi’s criminal family, and you watch that transformation happen from the inside. It’s an origin story setup: you see the early roots of organized crime in Sicily, before that legacy spreads to the United States. If you enjoy the classic Mafia Definitive Edition, this prequel connects to those foundational experiences.
The game is designed as an action-adventure with a third-person perspective. You move from one situation to the next, with the pacing controlled by the story rather than by an open world. If you liked the Mafia vibe for its scenes, characters, and pressure-filled missions, that’s the kind of direction this description points to.
Linear missions instead of an open world
Unlike previous Mafia games, Mafia: The Old Country does not give you an open world. That changes how it feels minute to minute. You’re not wandering around “just because.” You’re pushed forward. New locations show up when the story needs them. If you’re the type who likes tight mission design and clear goals, this can be a plus. If you mainly want free roaming, it’s worth knowing up front.

Movement and traversal
Because of the era and setting, you’ll see gangsters patrol on foot or on horseback as a normal thing. Some characters also have access to period cars. It’s a small detail, but it changes the flavor. Sometimes you’re moving like it’s the old days because… it is.
Combat: loud or quiet, knife or gun
When things go bad - or when you decide to make them bad - you have options. You can take enemies out in open fights, or you can go quiet and slip behind them. The game gives you both firearm combat and close-quarters tools.

- Knife combat is a real pillar, with Hangar 13 recreating a Sicilian fighting style for it.

Story and setting: early-1900s Sicily
The setting does a lot of the work here. Early-20th-century Sicily means different tools, different travel, and a different rhythm than later gangster stories. You’re watching a criminal family operate in its home environment. And you’re doing it as someone who wants out of a hard life, which makes the personal stakes easy to understand.
It’s also a clean entry point if you’re new to the series, since it’s described as an introduction to Mafia’s wider timeline. You’re starting at the beginning of this chapter, not catching up midstream.

You can pick between two digital editions. Standard is the base game. Deluxe Edition adds extra content if you like collectibles and bonus gear.
